In order to use the Employee Fitness Center, employees and medical staff members must send proof of being fully vaccinated against COVID-19 to Human Resources and complete the LADMC Fitness Center Agreement.
Complete these steps to apply to use the LADMC Fitness Center:

2. E-mail Human Resources your proof of being fully vaccinated against COVID-19 — hr@ladowntownmc.com
